PROVECTUS CARE GROUP PRIVACY POLICY

This is the privacy policy of Provectus Care Pty Limited, Beresford Hall Pty Limited and Victoria By The Park Pty Limited (“we”, “us” and “our”) and details how we collect, hold, use and disclose (“manage”) “personal information”, “health information” and “sensitive information” (as those terms are defined in the Privacy Act 1988 (Cth)) (“personal information”).
  2. We manage personal information in accordance with the Aged Care Act 1997 (Cth) (the Act), Privacy Act 1988 (Cth) and Australian Privacy Principles for the purpose of providing residential care and related services pursuant to the Act (“our services”).
  3. We collect personal information as required and authorised by, and for discharging our obligations pursuant to, the Act.
  4. If we are not able to collect your personal information, we will not be able to provide our services to you.
  5. The kinds of personal information that we collect and hold includes your name, address, health information, sensitive information, banking details, and the contact details of your “health service” providers (as that term is defined in the Privacy Act 1988 (Cth)), relatives, guardians and attorneys.  
  6. We collect your personal information from you, health service providers, relatives, guardians, attorneys, and other entities, bodies and persons in the course of providing our services to you.
  7. We take such steps as are reasonable in the circumstances to protect personal information that we hold from misuse, interference, loss, unauthorised access, modification and disclosure.  To the extent permitted by law, we accept no responsibility for loss or damage caused by any unauthorised access to your personal information.
  8. In the course of providing our services to you, we may disclose your personal information to the Commonwealth Government and other Commonwealth and state government bodies, to entities, bodies or persons that provide a health service, to your relatives, guardians and attorneys, unless you request us not to disclose personal information to a particular person, and to meet our legal obligations.
  9. We are not likely to disclose personal information to overseas recipients.
  10. If you want to know what personal information we hold about you, have any personal information corrected, or want to make a complaint, please contact us in writing and we will respond to your query within 14 days or another period reasonable in the circumstances:
